Menus that Liven Up Those Health Department Warnings

It's easy to ignore the legally mandated health warnings restaurants must post on the menu, especially given the tiny fonts, and the fact that some half-hearted words of caution aren't likely to dissuade anyone already hell-bent on ordering the geoduck carpaccio. However it's always nice to see some restaurants have a little fun while fulfilling this legal obligation.

Here are some examples from four local menus; see if you can identify all four restaurants. Answers are below.
